SouthLight Healthcare is a non-profit organization providing top quality care for substance use and mental health disorders as vital components of whole person care in the communities we serve. We have proudly served the Triangle area and beyond since 1970.

We are seeking a full-time Substance Use Professional to provide community-based services.

The primary role of the Community Support Team Substance Use Professional is to provide direct care services to identified clients, ensuring that clients receive optimal care and that services provided are consistent with Community Support Team service definition.

JOB RESPONSIBILITIES

Participate in clinical supervision from the team lead (monthly minimum)

Participate in required trainings as required (by service definition, Southlight, other governing bodies, and recommended by supervisor)

Provides psychoeducation as indicated in the PCP

Assist with crisis interventions

Assist the Team Leader with behavioral and substance use disorder treatment interventions

Assist with the development of relapse prevention and disease management strategies

Coordinates the initial and ongoing assessment activities

Conducts comprehensive substance abuse assessments considering the relationship between substance use and mental health.

Assesses, tracks and documents individual's stages of change readiness and stages of treatment.

Uses outreach and motivational interviewing techniques to work with individuals in earlier stages of change readiness.

Ensures that the team's treatment approaches are consistent with individual's stages of change readiness

Complete tasks specified in CST service definition NCMHDDSS Clinical Coverage Policy 8A https://ncdma.s3.amazonaws.com/s3fs-public/documents/files/8A.pdf

Provide clinically appropriate interventions and EBP to the CST caseload under the supervision of the team lead

Provide first responder services (rotation of crisis phone)

Provide case management services as necessary to clients on caseload

Provide assertive engagement to appropriate clients that may engage in CST

Complete documentation in a timely manner. Notes within 24 hours. PCPs and SARs (Service Authorization Requests) complete and submitted within allotted time frame to allow amply (14 day) turnaround time and assure no lapse in service authorizations occur.

Participate in outreach as necessary to build caseload in the community

Be fluent and knowledgeable about Assertive Engagement (AE) and CST service definitions and service requirements https://ncdma.s3.amazonaws.com/s3fs-public/documents/files/8A.pdf

Establish relationships with a comprehensive, targeted listed of providers and other relevant community organizations

Meet with clients in the community, homes, and offices as needed (remain within the location ratio specified in service definition)

Complete thorough transition plans for each client and assure a smooth transition to the next level of care

 

QUALIFICATIONS

Educational and Experience

Two years' experience working directly with Adults struggling with mental health and substance use disorders.

Two years' experience working in community mental health setting preferred

Qualified Professional status with MH/SUD required.

 

Licensure/Certifications

CCS, LCAS, LCAS-A or CADC - Certified Alcohol & Drug Counselor (formerly CSAC) required
